RESUMO

Phlebotomy, a therapeutic method of bloodletting typically performed using a needle, has a traditional technique known as "Fasd." In this method, blood is extracted by creating a longitudinal incision on a vein (3-5 mm) with a surgical scalpel blade, usually blade No. 11. Due to the incision in the vessel wall, establishing hemostasis is more challenging compared to conventional methods. Hemostasis is usually achieved within minutes after Fasd. We present a case highlighting an uncommon yet significant complication of traditional phlebotomy. A 55-year-old man with no prior medical conditions underwent traditional phlebotomy at an academic traditional medicine clinic. Senior MD-PhD students in Iranian Traditional Medicine, under professor supervision, performed Fasd. A sterile scalpel blade No. 11 was used to create a longitudinal incision of approximately 4 mm on the patient's median basilic vein in the right hand. After removing 400 cc of blood, a pressure dressing was applied to the incision site. Despite attempts such as hand elevation, ice pack application, prolonged direct pressure, and tight elastic bandaging, bleeding from the incision persisted. After an hour of supportive therapy, hemostasis was eventually achieved within a few minutes using burnt cotton dressing (a traditional method for blood hemostasis). Following intravenous hydration, the patient was discharged in stable condition and reported no issues during the one-month follow-up. The traditional phlebotomy (Fasd) carries the risk of serious complications, including uncontrolled and prolonged bleeding. Further research on the efficacy and safety of burnt cotton dressing for controlling hemostasis is recommended.

The golden age of Islamic medicine (800 to 1300 CE) is a notable period in medical history. Medical education in this period of time was significant and systematic in Islamic territory. In the early Golden Age of Islamic Medicine, Abu Zayd Hunain ibn Ishaq al-'Ibadi, an exceptional scholar and translator, emerged. He was known as Johannitius in medieval Europe. Al-Masa'il fi al-tibb lil-Mutallimin (Questions on Medicine for Students) was written by Hunain ibn Ishaq. This book remains a definitive text on Islamic medicine and has been printed and published widely in Europe. Al-Masa'il fi al-tibb is written in the style of questions and answers which is distinct from the conventional writing style of medical books on Islamic medicine. The current article reviews Al-Masa'il fi al-tibb and its distinguishing style, the question and answer format. Today, the question-and-answer method is a popular method of medical education, and clinical teachers tend to use it in medical education because of the advantages it offers. The use of this method in Al-Masa'il fi al-tibb for education and examination of medical students by Hunain ibn Ishaq reflects a great improvisation in medical education and introduces him as the leading developer of the question-and-answer method in Islamic medical education.

BACKGROUND: Hypoactive Sexual Desire Disorder (HSDD) is a common sexual problem of women which has negative impacts on their health and quality of life. Given the side effects of pharmacologic interventions, it would be beneficial to patients trying to find new options based on herbal medicine. OBJECTIVES: To evaluate efficacy of carrot seed on sexual dysfunction of women with HSDD compared with placebo. METHODS: In this randomized double-blind clinical trial, 68 participants randomly assigned to the intervention group which took 500 mg carrot seed three times a day for 12 weeks versus placebo. Participants in two groups filled Female Sexual Function Index (FSFI) questionnaire at baseline, week six and 12. Repeated measure analysis of variance (ANOVA) test was used for statistical analysis. RESULTS: Thirty women in carrot seed group and thirty women in placebo group completed 12 weeks of the study. In general, carrot seed compared to placebo improved the total score of FSFI 7.329 ± 0.830 (p < 0.001), desire 4.1±0.7 (p < 0.001), lubrication 4.7±0.4 (p = 0.019), arousal 4.1±0.08 (p < 0.001), satisfaction 4.8±1.1 (p < 0.001), orgasm 3.9±0.9 (p < 0.001) and pain 5.4±1(p < 0.001). No adverse event was reported in this study. CONCLUSIONS: Women with HSDD may benefit from six weeks' treatment with carrot seed for improvement of sexual dysfunction. Further large clinical studies are warranted to confirm efficacy of this herbal drug.

Galen (129-199 A.D.), an ancient Greek scholar, is one of the most influential and recognized physicians and surgeons in the history of medicine. He is one of the most influential Greek scholar in Persian medicine, who has been repeatedly cited by Iranian scholars. The purpose of this article is to describe the exact position of Galen in Avicenna's The Canon of Medicine, the most important work of Persian medicine. In The Canon of Medicine, Galen is cited about 300 times, and Avicenna always admired and praised Galen repeatedly. Galen's ideas were important to Avicenna, but there are also many elements of Galen's views that Avicenna challenged or criticized. For example, Avicenna diverged from Galen's philosophical view in medicine and rejected some of Galen's pharmaceutical opinions and Galen's views on the physiology of pain, the physiology of pulse, the same nature of the tendons and nerves and separation of the mind and body. Although the views of Greek scholars, especially Galen, in The Canon of Medicine have been quoted abundantly, Avicenna revised the knowledge of the ancient scholars through critical thinking and relying on observation and testing, systematized the science of medicine and introduced many ideas and innovations. As such, Europeans considered Avicenna as the most prominent physician of the Islamic Golden Age.

Ultrasound elastography is a non-invasive method which images the elasticity of soft-tissues. To make an image, pre and after a small compression, ultrasound radio frequency (RF) signals are acquired and the time delays between them are estimated. The first differentiation of displacement estimations is called elastogram. In this study, we are going to make an elastogram using the processing method named empirical mode decomposition (EMD). EMD is an analytic technique which decomposes a complicated signal to a collection of simple signals called intrinsic mode functions (IMFs). The idea of paper is using these IMFs instead of primary RF signals. To implement the algorithms two different datasets selected. The first one was data from a sandwich structure of normal and cooked tissue. The second dataset consisted of around 180 frames acquired from a malignant breast tumor. For displacement estimating, two different methods, cross-correlation and wavelet transform, were used too and for evaluating the quality, two conventional parameters, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) calculated for each image. Results show that in both methods after using EMD the quality improves. In first dataset and cross correlation technique CNR and SNR improve about 16 dB and 9 dB respectively. In same dataset by using wavelet technique, the parameters show 14 dB and 10 dB improvement respectively. In second dataset (breast tumor data) CNR and SNR in cross correlation method improve 18 dB and 7 dB and in wavelet technique improve 17 dB and 6 dB respectively.
